Maryland SF-2 Surgeon's Report (Workers' Comp)

Maryland's Workers' Compensation Commission requires the treating physician or surgeon to report on an injured worker within 72 hours after first treatment, then send progress reports and a final report at discharge. The SF-2 covers the accident as the patient describes it, the medical description of the injury, other contributing causes, pre-existing disease, delayed recovery, x-rays, hospitalization, return-to-work dates, permanent defect and disfigurement.

Who completes it

The treating surgeon or physician completes the whole report, including specialty, medical school and graduation year, and license state and number.

Submitting and payment

How to submit it
Submit it through the Commission's online Surgeon's Report form, where every field requires input; the printable PDF has no fillable fields.
Who pays
The SF-2 carries no fee; medical services are billed separately on the claim form the Commission lists, the CMS-1500.

Good to know

  • Mark whether each submission is the first, a progress or the final report; the instructions say not to fail to forward progress and final reports.
  • Item 10 asks about heart, lung, brain, kidney, blood or vascular disease, or any other disabling condition not due to the accident, with particulars.
  • Every question must be answered before the form is submitted. The printable version is dated July 2011.
  • Maryland permanency evaluations are narrative reports under COMAR 14.09.09 and have no Commission form.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
