Louisiana LWC-WC 1010A First Request for Information

If a Louisiana carrier or self-insured employer finds that a 1010 authorization request lacks the minimum information required by LAC 40:2715(C), it sends this one-page first request, checking which items are missing and explaining why. The health care provider responds in Section 3, either supplying the information or explaining why it was not provided. The authorization clock is suspended until the provider supplies it.

Who completes it

The carrier or self-insured employer completes Section 2. The health care provider completes Section 3, marking whether additional information has been provided and attaching it, or explaining why not.

Submitting and payment

How to submit it
The carrier faxes or emails it to the provider with the 1010, and the provider returns its response the same way; Section 4 of the 1010 records the dates of both.
Who pays
The form sets no fee.

Good to know

  • The checklist mirrors the 1010's required minimum information: history, physical findings and clinical tests, functional improvement from prior treatment, test or imaging results, and a treatment plan.
  • The posted PDF carries a DRAFT watermark; OWCA also offers an Excel version on its downloads page.
  • If information is still lacking, the carrier can suspend the prior authorization process, and the provider can appeal that suspension to OWCA Medical Services under 1010 Section 5.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What should a provider do after receiving a Louisiana 1010A?

Complete Section 3: mark that additional information has been provided and attach the supporting documentation, or mark that it has not and explain why. Then fax or email the response to the carrier and record the date in Section 4 of the 1010.

Does a 1010A pause the authorization clock?

Yes. A first request suspends the authorization clock until the provider supplies the missing minimum information, so the carrier's five business days to answer the 1010 do not run while the 1010A is outstanding.
