Virginia VWC Form 6: Attending Physician's Report

Virginia's Workers' Compensation Commission relies on this report for the treating physician's account of a work injury or occupational disease: how it happened, findings, diagnosis, treatment, periods of disability and any permanent defect. It answers whether the condition is related to the job and whether the patient has reached maximum medical improvement. The physician signs a certification of personal examination and treatment.

Who completes it

The treating physician completes and signs the whole report, including the patient's account of the injury, findings with X-ray and lab results, prior injuries and pre-existing conditions, referrals, disability dates and MMI status.

Submitting and payment

How to submit it
File it with the Commission through WebFile (an active account is required), by mail, or in person at any VWC office. Filings are accepted only in English.
Who pays
The Commission's form page and instructions do not set a fee for the report or say who pays for it.

Good to know

  • The Commission's instructions state plainly that the report must be signed by the treating physician, under a certification of personally examining and treating the patient.
  • Disability dates are split into when disability began, return to light duty and return to regular work, so give each date separately.
  • If there was a fracture or amputation, describe it; for hand or foot amputations, the physician may also complete the Commission's Amputation Chart.
  • Causation and permanent defect or disfigurement are answered yes, no or unknown; maximum medical improvement is yes or no with a date.

How this list is checked

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
