Virginia Workers' Comp Amputation Chart (VWC)

Virginia's Workers' Compensation Commission uses this one-page diagram to set permanent partial disability benefits after amputation of fingers, metacarpals, toes or metatarsals. The treating physician draws a straight line at the exact point of amputation on the hand or foot chart and certifies personally examining and treating the patient. It often accompanies the Attending Physician's Report (Form 6).

Who completes it

The treating physician completes the whole chart: claim numbers, patient and employer, injury and amputation dates, which hand or foot, the line marking and the signed certification of personal examination and treatment.

Submitting and payment

How to submit it
File it with the Commission by uploading it through an active WebFile account, by mail, or in person at a VWC office. The Commission accepts filings only in English.
Who pays
The Commission's form and instructions do not set a fee for completing the chart or say who pays for it.

Good to know

  • Circles are not accepted: the instructions require a straight line at the exact point of amputation so the Commission can calculate the correct permanent partial disability entitlement.
  • The diagram labels each finger and toe with its distal, middle and proximal phalanges, plus metacarpals 1-5 and metatarsals 1-5, so mark the exact bone involved.
  • Enter the Jurisdiction Claim Number (JCN) and the claim administrator number at the top so the chart is matched to the right claim file.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I circle the amputated area on Virginia's chart?

No. The Commission's instructions say circles are not acceptable. Draw a straight line across the finger, toe, metacarpal or metatarsal at the exact level of amputation, and indicate which hand or foot is involved, so the Commission can rate the permanent partial loss accurately.

Is the Amputation Chart filed on its own?

It can be, but the Commission's instructions for the Attending Physician's Report (Form 6) say that in hand or foot amputation cases the treating physician completes Form 6 and may also fill out the Amputation Chart, so the two are often filed together through WebFile.
