Missouri WIC-61 WIC Referral Form

Health care providers, social service programs and WIC staff use this one-page referral to connect a patient to Missouri WIC or other needed services. A physician's office can include current weight and length or height, birth measurements for infants, hemoglobin or hematocrit, lead results and reasons such as prematurity, poor growth or breastfeeding concerns.

Who completes it

The referring provider completes the patient details, measurements and reasons, then signs with printed name, address and phone. The patient, participant or guardian signs the acknowledgment at the bottom.

Submitting and payment

How to submit it
Fax the referral to the patient's WIC local agency or give it to the family to bring to the WIC appointment.

Good to know

  • WIC eligibility requires height and weight measurements and bloodwork, so sharing them on the referral helps the family. WIC can also measure and screen hemoglobin at certification.
  • A checkbox asks WIC to waive the physical presence requirement when the participant is medically fragile or hospitalized, with a projected discharge date no more than one week away.
  • Women's reasons include poor or excessive weight gain and breastfeeding concerns. Infant and child reasons include prematurity in weeks, small for gestational age, poor growth and lead results.
  • The patient acknowledges that WIC formula or food is for the participant only and cannot be used while the infant or child is still in the hospital.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can a Missouri WIC appointment happen while a baby is hospitalized?

The WIC-61 lets the referring provider ask WIC to waive the physical presence requirement because the participant is medically fragile or hospitalized, with a projected discharge date no more than one week away. The family acknowledges that WIC formula or food cannot be used while the child is still in the hospital.

Is the WIC-61 used to order special formula in Missouri?

No. The referral shares measurements and reasons for referral. Exempt formula, WIC-eligible nutritionals and food changes require the WIC-27 Medical Documentation form signed by a prescriptive authority, and Missouri WIC does not accept a regular prescription in its place.
