Missouri WIC-27 Medical Documentation Authorization

Missouri WIC requires this form before it issues exempt infant formula, WIC-eligible nutritionals or modified supplemental foods, or when contract formula must be mixed differently from the label. The provider records the formula, calorie concentration, daily amount, approval length and a qualifying condition tied to a Missouri WIC risk factor. A regular prescription is not an allowable substitute.

Who completes it

A state-licensed prescriptive authority (MD, DO, PA, NP, CNS or CNM) completes sections A to D and signs, and WIC staff complete all of section E. Local agency staff may fill in section C before the provider signs.

Submitting and payment

How to submit it
Send the form to the WIC local agency by fax, secure email or another secured electronic method, or have the participant hand-deliver it. A new WIC-27 is needed at least every six months.

Good to know

  • Signature stamps are not allowed. A verbal telephone order can cover one month of benefits if the signed WIC-27 reaches the agency within 1 to 2 weeks.
  • Qualifying conditions map to risk factors: low birth weight (RF 141), prematurity (RF 142), gastrointestinal disorders (RF 342), metabolic disorders (RF 351), severe food allergies (RF 353) and immune disorders (RF 360).
  • Ready-to-use formula needs a checked reason. Whole milk for women and children 24 months or older requires this documentation plus a WIC formula, and preference alone isn't allowed.
  • Health insurance, including MO HealthNet, pays first for special formulas. WIC is secondary when coverage is denied, the participant is uninsured or a coverage request is pending.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How long is a Missouri WIC-27 valid?

Approval length is 1 to 6 months and ends on the last day of the month. A new WIC-27 must be completed at least every six months, and some products, such as metabolic formulas, have shorter approval periods. WIC records the end date in section E.

When do I complete section C of the Missouri WIC-27?

Only when foods must be omitted or changed; otherwise full age-appropriate WIC food is issued. Section C asks whether an infant can eat infant foods or needs extra formula, whether a child or woman needs infant foods, and which foods to omit, such as cow's milk, eggs or peanut butter.
