Missouri DESE Medical Statement for Meal Modification

Missouri schools in the National School Lunch and School Breakfast Programs use this DESE statement when a student's disability or medical condition requires a meal modification, such as an allergy diet, a food substitution or a texture change. Federal rules, 7 CFR 210.10(m) and Section 504, require schools to accommodate disability-related diets, and this form documents the clinician's order.

Who completes it

A state-licensed healthcare professional, such as a physician, PA or APRN, or a registered dietitian completes the impairment or diagnosis, foods to omit and substitute, texture changes and signature. The parent supplies the student's details.

Submitting and payment

How to submit it
Give the completed statement to the parent, who returns it to the school food service director.

Good to know

  • DESE posts the statement under Most Requested Forms in the Food and Nutrition Services section of its website.
  • The form covers school meals. Missouri's Child and Adult Care Food Program is administered by the Department of Health and Senior Services, not by DESE.
  • The clinician's part asks for the impairment or diagnosis, the foods to omit, the foods to substitute and any texture modification, followed by the signature.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
