Wisconsin WIC F-44024D: Infants and Children

Wisconsin's F-44024D serves as both a WIC referral and a medical formula or food request for infants and children. Section I shares weight, length or height, birth data, hemoglobin or hematocrit, lead and human milk feeding status, which on its own supports WIC eligibility and remote services. Section II requests an approved formula for a qualifying condition, with the amount, length of use and any food restrictions.

Who completes it

The health care provider completes Section I for every referral and Section II when requesting formula; an MD, DO, PA or NP signs, and certified nurse-midwives with prescribing certification also qualify. A WIC registered dietitian nutritionist reviews and fills the request.

Submitting and payment

How to submit it
Fax or email the completed form to the patient's local WIC clinic; Wisconsin DHS posts a directory of local WIC agencies.

Good to know

  • Only the formulas printed on the form can be requested, and write-ins are not allowed. The Wisconsin WIC Formulary, P-40077, gives more product information.
  • Intended length of use can be 1, 3 or 6 months, or another number of months not exceeding 12.
  • For children, give ounces per day; ranges are allowed, but WIC max, ad lib and as tolerated are not. Infants typically receive the maximum amount WIC provides.
  • Pediatric drinks are not issued just for picky eating, refusing a multivitamin, or being underweight without a failure-to-thrive diagnosis.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I write in a formula on Wisconsin form F-44024D?

No. The form lists every Wisconsin WIC approved formula, from Enfamil AR and EleCare to Similac Alimentum, NeoSure and PediaSure Peptide 1.0, and states that write-ins are not allowed. Check the product you need and give the amount and intended length of use.

Is section 1 of F-44024D enough for a WIC referral?

Yes. Wisconsin DHS says only section 1 is needed when no medical formula or food is requested. Sending height, weight and hemoglobin lets participants receive remote services and reduces repetition at the WIC appointment. Complete both sections for a formula request.

Can Wisconsin WIC give whole milk to a child over 2?

Only with a medical formula and a diagnosis. The food restrictions section offers whole milk or whole milk yogurt for children 24 months or older only in combination with a medical formula, and the WIC dietitian assesses foods if the section is left unchecked.
