Wisconsin WIC F-44024A: Referral/Medical Food, Women

Wisconsin's F-44024A combines a WIC referral and a medical food request for pregnant, breastfeeding and non-breastfeeding postpartum patients. Section I carries weight, height, hemoglobin or hematocrit, due or delivery date and any need for lactation support; on its own it supports eligibility and lets the patient receive remote WIC services. Section II requests a listed medical food such as Ensure or Boost for a qualifying condition.

Who completes it

The health care provider completes Section I for any referral, and both sections when requesting a medical food; an MD, DO, PA or NP signs, and certified nurse-midwives with prescribing certification also qualify. A WIC registered dietitian nutritionist reviews and fills the request.

Submitting and payment

How to submit it
Fax or email the completed form to the patient's WIC clinic; Wisconsin DHS lists local WIC agencies online.

Good to know

  • ICD codes are not required, but a qualifying condition is: constipation, diarrhea, lactose intolerance and other intolerances are not accepted diagnoses.
  • Give the amount in bottles per day; ranges are allowed, but WIC max, ad lib and as tolerated are not.
  • Whole milk or whole milk yogurt is only available together with one of the listed medical foods and a medical diagnosis.
  • Length of use can be throughout pregnancy or 1, 3 or 6 months. The September 2025 edition is a Word form, and renewal is required periodically.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Do I need to fill out both sections of Wisconsin form F-44024A?

Only if you are requesting a medical food. Section I alone is enough for a referral: it shares the patient's measurements and hemoglobin or hematocrit, which helps WIC determine eligibility and serve the patient remotely. Complete every box in Section II when the patient needs a medical food.

Which medical foods can Wisconsin WIC provide for pregnant patients?

The form lists all approved products: Ensure Nutrition Shake, Ensure Plus, Ensure High Protein, Kate Farms Standard 1.0, Boost Original, Boost Plus, Boost High Protein and Boost Glucose Control. WIC cannot provide more than its maximum monthly amounts, which may not meet the patient's full needs.

Does Wisconsin WIC require ICD-10 codes on F-44024A?

No. The instructions say ICD codes are not required. Select a qualifying condition, such as an immune system disorder, gastrointestinal disorder, malabsorption syndrome or another condition that impairs nutrition status, and specify the diagnosis. WIC may ask for more documentation if it is missing, nonspecific or inconsistent with measurements.
