VA Form 10-10SH: State Home Medical Certification

A State Veterans Home must send this certification, with the Veteran's 10-10EZ or 10-10EZR, to the VA medical center of jurisdiction for each resident before VA pays per diem. The home's physician, APRN or PA records the history and physical, diagnoses, service connection, admission orders and the recommended level of care: skilled nursing home, domiciliary or adult day health care.

Who completes it

State Veterans Home staff complete Part I, and the SVH physician, APRN or PA completes and signs Part II. A registered nurse or the clinician signs the Part III evaluation, a therapist or clinician the physical therapy block, a social worker or clinician Part IV, and VA staff Part V.

Submitting and payment

How to submit it
The State Veterans Home submits the completed form to the VA medical center of jurisdiction together with VA Form 10-10EZ or 10-10EZR, on paper or electronically.
Who pays
The form is a condition of VA's per diem payment to the State Home, and an incomplete or incorrect form may delay or deny that payment.

Good to know

  • Item 44, the clinician's signature, cannot be signed until items 36 through 43 are filled in, and signing locks Part II as read-only.
  • Part II must contain enough medical information to justify the level of care being provided.
  • For domiciliary care VA reviews seven activities of daily living; if the Veteran can perform at least four, a VA clinician may waive the rest. The home must justify domiciliary care in Remarks.
  • Use the FEB 2025 edition; previous editions are not to be used.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who signs VA Form 10-10SH?

The State Veterans Home physician, APRN or PA signs Part II after entering the referring physician, diagnoses, service connection, recommended care and admission orders. Parts III and IV are signed by a registered nurse, therapist or social worker, or by the clinician, and VA reviewers sign Part V.

What levels of care can be recommended on the 10-10SH?

Item 41 offers skilled nursing home care, domiciliary care and adult day health care. VA's clinical reviewer then approves or declines the level of care in Part V, using the history, physical, functional evaluation and social work assessment the State Home provided.

Is VA Form 10-10EZ required with the 10-10SH?

Yes. The instructions say the State Home must submit a 10-10EZ (Application for Health Benefits) or 10-10EZR (Health Benefits Update) with the 10-10SH, on paper or electronically. Item 10 notes N/A applies to nursing home and adult day health care admissions for the financial disclosure question.
