SMUD-1519c: Medical Equipment Discount (MED) Rate

Sacramento Municipal Utility District offers a Medical Equipment Discount rate to households whose electricity costs rise because of qualifying home medical equipment or extraordinary heating or cooling needs. The customer completes sections 1 to 3, and a qualified health professional completes section 4 on the back. SMUD returns any application that lacks the health professional's signature.

Who completes it

The customer fills in account and mailing details and signs a declaration. Section 4 must be completed by a doctor of medicine, nurse practitioner, family nurse practitioner or physician assistant: yes or no for each device or need, name, office address, phone, license number and state, signature, and whether the need is permanent.

Submitting and payment

How to submit it
Mail the completed application to SMUD's MED Rate mail stop; SMUD takes questions by email or phone.
Who pays
The application lists no fee for the health professional's certification.

Good to know

  • Qualifying items are an electric wheelchair, in-home dialysis cycler, oxygen concentrator, ventilator, and extraordinary heating or cooling needs. CPAP and BiPAP machines do not qualify.
  • The customer consents to annual eligibility verification, and a box lets the professional mark the device as permanently required.
  • MED Rate can be combined with SMUD's Energy Assistance Program Rate, and MED customers are automatically included in SMUD's Vulnerable Population Program for planned-outage communication.
  • This is SMUD's own municipal program, separate from the CPUC Medical Baseline program used by investor-owned California utilities.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Does a CPAP qualify for SMUD's MED Rate?

No. The form and SMUD's page both say CPAP and BiPAP machines are not qualifying devices. A ventilator does qualify, as do an electric wheelchair, an in-home dialysis cycler and an oxygen concentrator, and a medical condition with extraordinary heating or cooling needs.

Who can sign section 4 of SMUD-1519c?

A doctor of medicine, nurse practitioner, family nurse practitioner or physician assistant licensed to practice medicine. The professional checks yes or no for each item, certifies the devices are required for the patient, and gives a license number and state. Applications without this signature are returned.

Is the SMUD MED Rate application available in Spanish?

Yes. SMUD posts fillable English and Spanish versions of the application on its MED Rate page. Both have the same four steps, with section 4 on the back reserved for the qualified health professional, who must sign it before the customer mails it.
