HACLA S 504-02: Reasonable Accommodation Verification

Housing Authority of the City of Los Angeles residents and Section 8 participants request disability accommodations, such as policy exceptions, unit modifications or changes to common areas, on Reasonable Accommodation Questionnaire S 504-02. Item 2 names a professional who can verify the disability and the disability-related need. HACLA then contacts that person directly; there is no published clinician form to download.

Who completes it

The head of household or co-head completes and signs the questionnaire, naming the household member, the accommodation needed and the verifier's contact details. The named professional, such as a licensed physician, physical therapist, psychiatrist, social worker, caseworker or counselor, answers HACLA's verification request.

Submitting and payment

How to submit it
The tenant returns S 504-02 to HACLA at application, at the annual review or at any time; HACLA then sends its verification request to the named professional, who responds as HACLA directs.
Who pays
HACLA's published forms list no fee or payment for the professional's verification.

Good to know

  • Item 1c covers requests to rescind a negative action taken because a household member's disability prevented compliance with program rules, with an explanation and any dates.
  • Requesting an accommodation does not affect program participation, and the questionnaire can be submitted at any time.
  • The policy, procedure and forms are available in English, Spanish, Armenian and Russian. S 504-08 is HACLA's grievance form.
  • For Section 8 tenants, owners approve unit modifications; if an owner refuses, HACLA explains how to file a housing discrimination complaint.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Where is HACLA's verification form for doctors?

HACLA does not publish one. The tenant names a verifier in item 2 of S 504-02, and HACLA contacts that professional directly to verify the disability and the disability-related need for the accommodation. Respond to the request HACLA sends rather than looking for a downloadable form.

Who can verify a disability for HACLA?

Item 2 lists examples: a licensed physician, physical therapist, psychiatrist, social worker, caseworker or counselor. The list is not exhaustive, so another professional who knows the person's disability and the need for the accommodation can be named, with agency, address, phone, fax and email.
