Utah Form 1GA General Assistance Evidence of Impairment

Utah's General Assistance program gives time-limited cash help to single adults and couples without dependent children who cannot work. Applicants must verify an impairment that prevents basic work activities in any occupation, meaning they cannot work any hours, and that lasts at least 60 days from the application date. Form 1GA is how a health care professional verifies it.

Who completes it

A health care professional who meets the credentials in DWS policy, such as a physician, APRN, physician assistant or psychologist, completes and signs the verification. DWS issues the form for the applicant's General Assistance case.

Submitting and payment

How to submit it
There is no public download; DWS issues Form 1GA for the individual case. The completed form goes back to DWS through a myCase upload, by mail or by fax to eligibility services.

Good to know

  • The impairment must prevent basic work activities in any occupation, not just the person's usual job; DWS describes this as being unable to work any hours at all.
  • It must be expected to last 60 days or longer from the date of application.
  • General Assistance benefits are time limited and begin the first day of the month after the application month, not the application date.
  • Eligibility also depends on household income and resource limits set by DWS, which the form does not address.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
