DC SNAP Work Requirement Medical Form (SNAPWR02.01)

DC's Department of Human Services uses this three-page form to excuse SNAP participants from work requirements. A qualified professional confirms a physical or mental condition that reduces the ability to work, with expected duration, or a pregnancy, or regular participation in substance use treatment or recovery services. DHS introduced the form in June 2026 with the expanded federal SNAP work rules.

Who completes it

A wide range of qualified professionals can complete Sections 3 and 4, from physicians, PAs, NPs and psychologists to therapists, counselors, community health workers and substance use counselors. The participant fills in Section 1 and signs the release in Section 2.

Submitting and payment

How to submit it
The participant or professional returns it to DHS's Economic Security Administration through District Direct (online or the mobile app), by mail, by fax or at a DHS service center, keeping a copy.
Who pays
The form sets no fee and does not say who pays for completing it.

Good to know

  • Expected duration is checked in bands from less than 30 days through over 12 months or permanent, counted from the date the form is completed.
  • For substance use treatment or Recovery Support Services, the professional gives the program start date and anticipated end date.
  • DHS will not accept the form without the professional's signature, date and title or profession, plus the attestation that they can provide competent medical evidence.
  • The participant's authorization lasts until their DC SNAP participation ends and can be revoked in writing at any time.
