Massachusetts EAEDC Medical Provider Statement (DTA)

Emergency Aid to the Elderly, Disabled and Children (EAEDC) is Massachusetts' state-funded cash assistance, and people applying on the basis of disability need this statement. A licensed clinician reports whether a disability or health condition, alone or combined with others, is expected to last 60 days or more and substantially reduces or eliminates the person's ability to support themselves through work.

Who completes it

A licensed clinician who examined the patient in the last 30 days, or who is familiar with the patient's condition, completes everything after the patient authorization: conditions, duration and functional limitations, then signs. The patient signs the authorization at the start.

Submitting and payment

How to submit it
Give the completed statement to the patient to return to DTA, or send it directly to DTA.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

Who can complete the EAEDC Medical Provider Statement?

A licensed clinician who has examined the patient within the last 30 days or is familiar with the patient's condition. They describe the conditions, how long they are expected to last and the functional limitations, then sign the statement for DTA.

Is the EAEDC Medical Provider Statement available in Spanish?

Yes. DTA publishes a Spanish edition, EAEDC-MPS-SP (04-209-1222-05), which translates the patient authorization. The provider section of the Spanish edition stays in English, so the clinician completes the same questions either way.

How long must the condition last for EAEDC?

The statement asks whether the disability or health condition, alone or combined with others, is expected to last 60 days or more and substantially reduces or eliminates the person's ability to support themselves through work. DTA decides eligibility using that answer and the limitations described.
