South Dakota DSS-CP-506 Foster/Adoptive Health Report

South Dakota Child Protection Services asks foster and adoptive parent applicants for this two-sided health report. The clinician's opinion on whether the applicant is free from physical or mental illness that might be detrimental to the care of children is described on the form as a governing factor in approval, and the information is used only for licensing or approval.

Who completes it

A physician, physician assistant or certified nurse practitioner completes Part A, the physical exam section, and signs. The applicant may complete Part B on the back, the immunization record for their own children under 18, and signs it.

Submitting and payment

How to submit it
Return the report to the DSS office written in the 'Please return to' box at the top of the form.

Good to know

  • A physical exam given within one year before the application is acceptable, so a recent annual visit can support the report.
  • Part A asks whether the applicant is under treatment for chronic illness and the diagnosis, prescribed medications and general health.
  • The key question is whether emotional, mental or physical factors would interfere with the person's ability to care for children in their home, with an explanation.
  • South Dakota law allows medical and religious exemptions for the applicant's own children's immunizations; the applicant tells the licensing worker. Edition DSS-CP-506 03/09.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who can complete the South Dakota foster parent health report?

Part A must be completed by a physician, physician's assistant or certified nurse practitioner. The clinician prints their name, signs as MD, PA or CNP and dates the form after answering the questions on chronic illness, medications, general health and factors affecting child care.

Can I use an exam from earlier in the year for DSS-CP-506?

Yes. The note to the medical professional says a physical exam given to the applicant within one year before the application is acceptable for meeting the requirement. Enter the date the applicant was seen at the top of Part A.

What is Part B of DSS-CP-506?

An immunization record for the applicant's own children under 18, which the applicant may complete with dates for polio, Hib, MMR, hepatitis B and DTP and then sign. South Dakota law allows medical and religious exemptions, which are raised with the licensing worker.
