Hawaii DHS 915 Caretaker Disability Report (Child Care)

Hawaii pays for child care when a parent or adult caretaker cannot care for their children because of a disability, and the Benefit, Employment and Support Services Division uses this report to document it. The examiner gives the diagnosis and history, whether the disability is permanent, whether the applicant can cope with caring for the children, a projected date to resume care and a prognosis.

Who completes it

An examining physician, psychologist or psychiatrist completes Section III and signs Section IV. The applicant completes Section I, listing the children, and signs the Section II consent, which also lets the Child Care Payment Worker discuss the report with the examiner.

Good to know

  • A report is valid for no more than 12 months; a new one is needed for each 12-month eligibility period, and the form is marked as a new application or 12-month recertification.
  • The form asks examiners to complete each item and write 'none' or 'n/a' where appropriate.
  • Diagnosis and history are broken out into chronic or handicapping emotional or physical conditions, drug abuse and other illness.
  • If the examiner answers no on the applicant's ability to cope with caring for the children, they must specify the health history and other factors behind it.
