Wisconsin W-2 DWSP10786 Need to Care for Family Member

When a Wisconsin Works (W-2) participant says they must stay home to care for a disabled or incapacitated family member, the W-2 agency asks a medical provider or agency to confirm it on this one-page form. The provider describes the disability, the personal care it requires and whether a child or adult facility could provide that care, and sets how long the restriction lasts.

Who completes it

The family member's medical provider, or an agency involved in their care, completes the whole form and signs it with name and title. The W-2 agency enters the case name and number.

Submitting and payment

How to submit it
The completed form goes back to the W-2 agency, which keeps it in the participant's case record.
Who pays
The form sets no fee and does not say who pays the provider for completing it.

Good to know

  • Describe the extent of personal care in practical terms: daily care needs, behavior patterns, need for supervision and therapeutic needs are the examples printed on the form.
  • The provider checks whether care can be provided in a child or adult facility, and if not, explains why the family member cannot be cared for there.
  • The restriction runs until an end date the provider enters or until a re-evaluation date, so one of the two dates should always be filled in.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who completes DCF-F-DWSP10786?

A medical provider or agency familiar with the disabled family member completes it. It records the family member's name and date of birth, the nature of the incapacity, the personal care required, whether facility care is possible, how long the restriction lasts, and the provider's title and signature. The W-2 agency adds the case name and number.

How long does a W-2 need-to-care statement last?

The provider decides. The form states the restriction remains in effect until a date the provider enters, or until the patient is re-evaluated on a set date. The W-2 agency keeps the signed form in the participant's case record.
