Wisconsin Shares DCF-F-5566 Parent Medical Exemption

Wisconsin Shares child care subsidies require every parent in the household to take part in an approved activity such as work or school. In a two-parent or three-generation household, this voluntary form lets a provider document that one parent's disability or health condition leaves them unable to care for the children, unable to take part in an approved activity, or both, and for how long.

Who completes it

A doctor, physician assistant, nurse practitioner, psychiatrist or psychologist completes the Medical Information section and signs. The parent fills in their contact details, and local agency staff complete the agency section.

Submitting and payment

How to submit it
The parent returns the signed form to the local agency in person, by fax or by mail, or the provider can fax, email or mail it to the agency directly.
Who pays
The form sets no fee and does not say who pays the provider for completing it.

Good to know

  • The form is voluntary: under Wis. Admin. Code DCF 201.039(4)(a), the same information can come on medical facility letterhead or in an email sent directly by the provider.
  • Mark whether the condition is permanent or temporary; for a temporary condition, give begin and end dates for the period the parent cannot participate or care for the children.
  • Use blue or black ink and leave the shaded areas blank. The provider checks a qualification box such as medical doctor, nurse practitioner, licensed psychologist or psychiatrist.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Who can sign the Wisconsin Shares Parent Medical Exemption?

Wisconsin Administrative Code DCF 201.039(4)(a) accepts documentation from a doctor, physician assistant, nurse practitioner, psychiatrist or psychologist. The form has checkboxes for those qualifications plus an other option, and the provider prints their name and address, signs and dates the medical section.

Does the provider have to use form DCF-F-5566-E?

No. The form is voluntary. The required information can be provided on medical facility letterhead or in an email sent directly from the provider to the local agency, as long as it shows whether the parent can care for the children or take part in an approved activity and how long the condition lasts.

What counts as an approved activity for Wisconsin Shares?

The form defines an approved activity as employment, education or other activities that lead to work. All parents must take part in one to receive child care subsidies, which is why the provider marks whether the condition prevents participation, child care, or both.
