Wisconsin DCF-F-4070 At Risk Pregnancy Verification

Wisconsin Works (W-2) offers an At Risk Pregnancy placement with payment and services for an unmarried pregnant woman who has no minor children in her custody and cannot work because of a high-risk pregnancy. This form is the medical verification: the physician confirms the due date, that the pregnancy is high risk and that it prevents work, based on an exam within four months of the due date.

Who completes it

The patient's physician completes the whole form after examining her, including due date, high-risk status, cause, start date of inability to work, specialty, NPI and signature. The W-2 agency fills in its contact details at the top.

Submitting and payment

How to submit it
The physician returns the completed form to the W-2 agency listed at the top of the form.
Who pays
The form sets no fee and states that the information provided will not affect billing or reimbursement from Medicaid.

Good to know

  • The placement requires the woman to be in her third trimester, judged from the due date, so the due date on the form is central to eligibility.
  • A letter on the physician's letterhead may be used instead, but only if it covers all of the same items the form asks for.
  • If the inability to work started before the signature date, the form asks for that earlier start date, and the physician lists the cause of the high-risk status.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What does the physician have to confirm on DCF-F-4070-E?

Three things: that the patient is in the third trimester based on her due date, that the pregnancy is high risk, and that the high-risk pregnancy leaves her unable to work, in the physician's best determination. The physician also states the cause of the high-risk status and when the inability to work began.

Can a physician letter replace the W-2 At Risk Pregnancy form?

Yes. DCF accepts all of the same items written on the physician's letterhead instead of the form. The letter still has to come from the patient's physician and reflect an examination within four months of the due date, and it goes to the same W-2 agency.

Who qualifies for the W-2 At Risk Pregnancy placement?

An unmarried pregnant woman with no dependent minor children in her custody at home, who has medical verification of a third-trimester, high-risk pregnancy that prevents her from working. The W-2 agency makes the eligibility decision; the physician supplies only the medical verification.
