Tennessee DCS CS-4286 Medical Report

Each health visit for a child in Tennessee DCS custody, from a comprehensive exam or well-child check to medical or dental treatment, therapy or lab work, is reported on this form. The provider records the service, vitals, reason for visit, instructions for the caregiver, any referable conditions and follow-up appointments so DCS can track the child's care.

Who completes it

The child's health care provider (medical, dental or behavioral) completes and signs the form with clinic name and address. The child's name, DOB, ERS ID number and home county identify the case.

Submitting and payment

How to submit it
Give the report to the caregiver or DCS case manager; the completed form is forwarded to the DCS regional Health Unit within 5 business days, using the county list on page 2.

Good to know

  • Do not use CS-4286 for psychotropic medications; those visits go on CS-0629, Psychotropic Medication Evaluation.
  • The form asks whether the service is ongoing (with frequency), whether there are referable conditions, and whether appointments have been made for them.
  • Special instructions to the caregiver and the return-to-clinic date help the foster family follow the plan between visits.
  • The current edition is CS-4286 Rev. 7/25; a Spanish version, CS-4286-SP (Informe Medico), is also posted.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
