Tennessee DCS CS-0678 Foster Parent Medical Report

Tennessee DCS uses this one-page report for foster parent applicants and other adults living in the home. The person first records smoking, alcohol, medical, vaccine and mental health history; a medical professional then documents TB risk or test results, conditions that would affect caring for a child, and whether they recommend the person as a foster or adoptive parent.

Who completes it

A physician, nurse practitioner or physician assistant completes the 'To be completed by medical professional' section and signs. The foster parent or other household adult completes the smoking, medical and mental health history above it.

Submitting and payment

How to submit it
Return the report to the DCS or private agency resource parent worker; it is filed in the foster home case record.

Good to know

  • TB can be documented by a risk assessment with date and results, a TB (PPD) test with date and results, or by marking the person not at risk or low risk.
  • For other adults in the home, note any conditions detrimental to a child's placement in the home, not only the person's ability to parent.
  • A checkbox lets you state you are not the person's primary care provider and are reporting from a single exam plus what they told you that day.
  • The applicant section asks for the last influenza immunization and adult pertussis vaccine dates. Current edition: CS-0678 Rev. 06/23, with a Spanish version.
