Tennessee DCS CS-0629 Psychotropic Med Evaluation

For children in Tennessee DCS custody, the prescriber records every psychotropic medication evaluation on this form, or provides their own documentation with the same information. It lists the DSM-5 diagnosis, symptoms, other treatments, each current or discontinued medication with dose, frequency, route and refills, the reasons for changes, vitals, labs and the next appointment.

Who completes it

The prescribing psychiatrist, physician or nurse practitioner completes and signs the form at each medication evaluation appointment. It also records the child's name, DOB, Electronic Record System ID, home county and family service worker (FSW) name, plus the provider and clinic details.

Submitting and payment

How to submit it
Forward the completed form to the DCS regional Health Unit for the child's county, listed on page 2 by region with child health email boxes; it is distributed to the case file and health record.

Good to know

  • If a new psychotropic medication is prescribed, form CS-0627, Informed Consent for Psychotropic Medication, must be signed, attached and forwarded to the DCS Health unit.
  • Each medication line is marked increase, decrease, discontinued, no change or new, with spaces for the reason a medication stopped and the reason for changes.
  • PRN psychotropics need prior approval on a separate form, CS-0628. CS-4286 Medical Report should not be used for psychotropic medication visits.
  • The current edition is CS-0629 Rev. 3/25; DCS says to use the version on its Forms webpage and not to alter the form without approval.
