Iowa HHS 470-0720 Health Report for Resource Families

Iowa requires prospective foster (resource) family members to show they are physically and mentally able to care for children before the home is licensed. The recruitment and retention contractor sends this form to the family's practitioner, who states whether the family's health would keep them from providing the needed care or would be detrimental to a child placed with them.

Who completes it

The examining physician or other practitioner completes Section B. The contractor completes Section A, addressed to the practitioner, and the family completes the health history in Section C and signs the release (a parent signs for a minor child).

Submitting and payment

How to submit it
Return the completed form as directed by the contractor or HHS worker named in the release; the contractor keeps a copy and sends the original to HHS in the licensing packet.

Good to know

  • A separate form is needed for each practitioner when family members are under the care of different physicians.
  • The form is not posted as a public PDF; HHS staff print it from the employees' manual, so the family or contractor supplies the copy.
  • It is completed before the home is licensed, and the practitioner or supervising agency can require a medical re-examination later.
  • It was renamed in June 2023 from Health Report for Foster and Adoptive Parents, so older copies may carry that title.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Where do I get Iowa form 470-0720?

From the family or the recruitment and retention contractor handling their licensing. Iowa HHS staff print it from the employees' manual or internal SharePoint; it is not posted as a standalone public download. The contractor fills in Section A before it reaches the practitioner.

What does the practitioner decide on the Health Report for Resource Families?

Whether the family's health would prevent them from providing the care a foster child needs, or would be detrimental to the well-being of a child placed in their care. The practitioner records this in Section B, alongside the family's reported health history in Section C.
