Vermont DCF 210A Training and Employment Medical Report

Vermont's Economic Services Division (ESD) uses Form 210A to get a medical provider's report on a participant's ability to take part in training and employment activities. In programs such as Reach Up and 3SquaresVT, that report supports decisions to defer a participant from work requirements or to modify them. The finished report goes back to ESD, where the case manager uses it in the participant's plan.

Who completes it

The participant's medical provider completes and signs the report on the participant's ability to take part in training and employment activities.

Submitting and payment

How to submit it
The completed report goes back to DCF's Economic Services Division through the ESD Document Uploader, by mail, or at a local district office.
Who pays
The ESD forms listing sets no fee for the report and does not say who pays the provider.

Good to know

  • Form 210A appears on the Economic Services Division's applicant forms page among the supplemental forms, next to Form 210PO, Exemption Personal Obstacle or Barrier.
  • The ESD Document Uploader accepts documents securely from a phone, tablet or computer, so a signed report can reach ESD without a visit to a district office.
  • Questions about what a Reach Up participant needs documented go to the participant's Reach Up case manager or the DCF Benefits Service Center.

How this list is checked

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
