California LIC 701A G-Tube Care Physician Checklist

California child care facilities licensed by Community Care Licensing, including centers, schools and family child care homes, need LIC 701A before staff give a child gastrostomy-tube feeding or liquid medication through a G-tube. The physician confirms the child's condition is stable enough for trained laypeople, designates who will teach them, and writes step-by-step instructions.

Who completes it

The child's authorized representative signs Part A, the consent and release of information. The physician, physician assistant or nurse practitioner completes Part B, the medical assessment and written instructions, and signs.

Submitting and payment

How to submit it
Return the completed checklist to the child care facility through the parent or authorized representative, attaching the written instructions on extra sheets.

Good to know

  • The instructions must cover activity limits, feeding frequency and formula, hydration, method and syringe height, positioning, side effects, emergencies, flushing, stoma care and storage.
  • Instructions must be updated annually or whenever needs change, for example a new type of G-tube or a change in formula, amount or frequency.
  • Decompression is not to be performed beyond briefly removing the cap from the gastric feeding button, as the form states.
  • For a child attending a child care center, a completed LIC 701 Physician's Report may be attached for the medical assessment section.

Frequently asked questions

Who can train child care staff on G-tube care in California?

Each person who gives G-tube care must be instructed by a competent person designated by the child's physician. The physician names that person on LIC 701A, and it may be the child's authorized representative, such as a parent.

How often must LIC 701A instructions be updated?

At least annually, and whenever the child's needs change, such as a different type of G-tube or a change in the frequency of feeding or the amount or type of formula or liquid medication.
