Illinois CFS 602 Medical Report on Child Care Adult

Illinois DCFS requires a medical report for employees and volunteers in licensed child care facilities, operators of day care and group day care homes, and other adults in their households. CFS 602 records a tuberculin test, the immunization discussion, findings and the physician's recommendation on fitness. Re-examinations are logged on page 2.

Who completes it

The examining physician, APN or PA completes Sections I to III, signs, and gives a state license number. The top of the form identifies the person examined, their position and the facility.

Submitting and payment

How to submit it
Give the completed report to the employee or volunteer, who files it with the licensed facility.

Good to know

  • The Mantoux tuberculin test, or chest X-ray for a positive reactor, is required in the initial exam only; the physician decides on testing at later exams.
  • For facilities caring for children age 6 and under, the examiner checks whether the person received 1 dose of Tdap and 2 doses of MMR (or is immune), or is not medically indicated.
  • The examiner notes any condition that rules out serving as a food handler or child care facility driver.
  • The examiner marks which age groups the person could physically care for: 0 to 2, 2 to 6, 7 to 12 or 12 to 18.
