Pennsylvania CD 322 Child Care Staff Health Assessment

Pennsylvania child care regulations (55 Pa. Code 3270.151, 3280.151 and 3290.151) require a health assessment for staff at initial employment and every two years. The employer lists the job's duties, and the examining clinician reports whether a physical was done, any communicable disease and whether the person is suitable to provide child care, with TB testing recorded at hire.

Who completes it

The employer completes the section describing the job's activities. A physician, physician assistant or certified registered nurse practitioner completes the examination, suitability and TB sections and signs.

Submitting and payment

How to submit it
The completed assessment is given to the employee for the child care facility's file.
Who pays
The form sets no fee and does not say whether the employer or the employee pays for the assessment.

Good to know

  • The exam should include functional vision and hearing and a systems review for conditions affecting performance or occupational injury, including hand washing, stress and exposure to childhood infections.
  • The form notes substance abuse should be considered in judging suitability, and a no answer requires details of conditions that might threaten children's health or safe care.
  • TB testing by Mantoux or IGRA is required at initial employment; later testing is needed only if directed by a clinician, the Department of Health or a local health department.
  • A positive test with a negative chest X-ray needs no further testing unless the person is exposed to active TB or has a productive cough not responding to treatment within 14 days.

Frequently asked questions

How often do Pennsylvania child care staff need a health assessment?

The CD 322 is used at initial employment in child care and again for the biennial re-examination; the clinician checks which reason applies. TB testing is required only at initial employment unless a clinician or health department directs more.

Who can sign Pennsylvania form CD 322?

The examination section is completed by a physician, physician's assistant or certified registered nurse practitioner (CRNP), who signs with date, title, printed name, address and telephone. The employer completes a separate section on the job's duties.

What job duties does the CD 322 ask the employer to list?

The employer checks lifting and carrying children, close interaction with children, food preparation, desk work, driving vehicles, facility maintenance or other duties. The clinician uses these when judging conditions that might affect performance or cause occupational injury.
