Colorado Asthma Care Plan and Medication Order

Colorado schools and child care programs use this one-page statewide plan both as the student's asthma care plan and as the health care provider's order for the quick-relief inhaler. Green, yellow and red zones set pretreatment before strenuous activity, dosing for mild symptoms and emergency steps with a call to 911. The plan is good for 12 months unless district policy says otherwise.

Who completes it

The parent or guardian completes and signs the top section. The student's physician, NP or PA completes all provider items (quick-relief medication, triggers, self-carry choice, pretreatment and zone puffs) and signs; the school nurse or child care health consultant signs last.

Submitting and payment

How to submit it
Give the completed plan to the parent, who returns it to the school nurse or child care provider along with prescribed, non-expired medication and a spacer.

Good to know

  • The provider chooses whether the student needs supervision with the inhaler or can self-carry; self-carry also needs school nurse approval and a completed self-carry contract.
  • Pretreatment can be marked not required, at student or parent request, or routine, with 2 or 4 puffs 10 to 15 minutes before strenuous activity.
  • For a student with a life-threatening allergy, the red zone refers staff to the anaphylaxis care plan, so the Colorado allergy and anaphylaxis plan belongs on file too.
  • The form, revised February 2021, also covers reactive airways, exercise-induced bronchospasm and twitchy airways. Children's Hospital Colorado offers a parent asthma intake form in English and Spanish.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. Frequently asked questions

How long is the Colorado asthma care plan valid?

The form says it is good for 12 months unless specified otherwise in district policy. It serves as both the care plan and the provider's medication order, and the parent's signature approves the plan and commits the family to supply prescribed, non-expired medication and supplies such as a spacer.

Can a Colorado student self-carry an asthma inhaler?

Yes, when the provider checks that the student understands proper use of asthma medications and can self-carry and use the inhaler independently. The form makes that subject to approval from the school nurse and completion of a self-carry contract, which CDE posts along with a student self-carry readiness tool.
