Colorado Seizure Action Plan (Children's Colorado)

Colorado's Department of Education links this two-page Seizure Action Plan, published by Children's Hospital Colorado (2025 edition), as its sample plan for students with seizure disorders. Since HB21-1133, Colorado K-12 schools train staff in seizure recognition and first aid and use individualized seizure action plans under rule 1 CCR 301-112. The student's provider completes the medical portion, and the parent returns the plan to school.

Who completes it

The student's physician, NP or PA completes the seizure description, rescue medication orders and provider signature, working with the parent. CDE guidance says the school nurse or health-care practitioner creates the student's individualized plan with the parent and, as appropriate, the physician.

Submitting and payment

How to submit it
Give the signed plan to the parent, who returns it to the school nurse or child care provider; CDE encourages parents to submit it at the start of the year or enrollment, soon after diagnosis, or when the condition changes significantly.

Good to know

  • CDE guidance lists what a plan should hold: seizure type and triggers, home treatments, 504 or IEP history, when to call 911, 'if you see this, do this' steps, rescue orders and a provider signature.
  • Once a school receives a parent-signed plan or learns of a diagnosis, it must have a designated employee trained to give seizure rescue medication and manual vagus nerve stimulation within 8 weeks.
  • Schools must follow their medication policy in approving the plan, in line with Colorado's Nurse and Nurse Aide Practice Act.
  • Children's Colorado publishes headache and anxiety action plans alongside it, and CDE also links Epilepsy Foundation seizure forms, including a parent questionnaire.
