TRS Texas TRS 58: Attending Physician Statement

Texas public school employees applying for disability retirement through the Teacher Retirement System of Texas need their attending physician to complete TRS 58. The TRS Medical Board reviews it with the member's statement and clinical evidence such as medical history, diagnostic tests and laboratory results, then decides whether to certify that the member is disabled from further performance of duty and that the disability is probably permanent.

Who completes it

The member's attending physician completes the whole statement. The member completes TRS 59, the Application for Disability Retirement, and TRS 59A, the Statement of Member in Claim for Disability Retirement.

Submitting and payment

How to submit it
The statement is returned to TRS with the disability retirement packet for Medical Board review. It is not on TRS's public forms page: TRS mails the retirement packet within 60 days after the member requests an estimate with TRS 18.

Good to know

  • The Medical Board certification has two parts: the member is mentally or physically disabled from further performance of duty, and the disability is probably permanent.
  • TRS lists TRS 58 and TRS 59A, the member's own statement, as the two forms required for Medical Review.
  • Members can apply regardless of age or years of service credit; those with at least 10 years receive a monthly annuity that is not reduced for early age.
  • Members start by requesting an estimate on TRS 18. TRS explains the steps in its Applying for Disability Retirement guide, revised June 2023.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
