TCDRS-32 Physician's Statement: Texas County Plans

Members of the Texas County & District Retirement System applying for disability retirement ask their attending physician to complete this three-page statement for the TCDRS Medical Board. It records vitals, diagnosis, onset, etiology, severity and course, treatment response, abnormal findings by system, physical and mental limitations, and a certification on whether the member is incapacitated for any gainful occupation, likely permanently.

Who completes it

The member completes Member Information and signs the Member Authorization. The attending physician completes Physician Information, Medical Condition(s), Physician Certification and Physician Authorization.

Submitting and payment

How to submit it
Return the statement to TCDRS by mail with photocopies of a current history and physical, hospital discharge summary, narrative report or comprehensive consultation, and other pertinent records.

Good to know

  • TCDRS entitlement requires mental or physical incapacity for any gainful occupation that is likely to be permanent, a broader test than the member's own job.
  • The certification also asks whether the incapacity is due to an injury sustained during the performance of duty for a TCDRS participating employer.
  • Attached records should reference every item on the form, since actively practicing physicians on the Medical Board interpret them.
  • Any corrections or whiteouts must be initialed. TCDRS covers about 800 Texas counties and districts, and the current revision is 08/2026.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

What standard does the TCDRS Medical Board use?

Under the TCDRS Act, a member is entitled to disability retirement if they are mentally and/or physically incapacitated to engage in any gainful occupation and the incapacity is likely to be permanent. The physician certifies whether the member's condition clearly comes within that provision.

What should be attached to TCDRS-32?

Photocopies of a current history and physical, a hospital discharge summary, a narrative report or comprehensive consultation, and other pertinent information. TCDRS asks that the attachments contain references to all items on the form so the Medical Board can review them.
