NYSTRS RET-54.3 Medical Report: Disability Retirement

New York State Teachers' Retirement System members applying for disability retirement give a copy of this Medical Report to each physician involved in their care. The physician gives treatment and onset dates, answers whether the member is totally and permanently disabled from all gainful employment, and writes a narrative of history, treatment, diagnosis and prognosis for the NYSTRS Medical Board.

Who completes it

The member completes and signs Part 1, the authorization, and forwards the form. Each physician completes Part 2, the dates and the total and permanent disability questions, and Part 3, the narrative, signing both and noting specialty and board certification date.

Submitting and payment

How to submit it
The physician sends the report to NYSTRS with a comprehensive history, diagnostic test results such as X-ray, MRI and CAT scan reports, surgical and pathology reports, and a detailed narrative of current health status.

Good to know

  • NYSTRS's standard is strict: total disability means incapacity for any gainful employment, and permanence means the end of the disability cannot be foreseen for at least one year.
  • Total disability is not considered permanent if the member could alleviate or control the condition with a standard treatment that is not inherently dangerous.
  • Physicians without access to the records can sign and date the form and forward it to their medical records department or facility to attach the documentation.
  • Tier 4 to 6 members must file within 12 months of termination, and members need 10 years of New York service credit unless the disability came from an on-the-job accident.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

What is the NYSTRS disability standard?

The member must be totally and permanently disabled at the time they ceased performing duties. Totally disabled means physically or mentally incapacitated from gainful employment, and permanently means the end of the disability cannot be foreseen for at least one year. The member carries the burden of proof.

Does the NYSTRS Medical Board examine the member?

The file is presented at the Medical Board's monthly meeting, and the Board can recommend an examination by a physician selected by the System. Refusing a required exam is grounds to deny or discontinue disability retirement benefits.

Where do I find the RET-54.3 form?

It is part of the NYSTRS Disability Retirement Application package, which includes three copies of the Medical Report along with the RET-54.1 application and the RET-54.1B Medical Information Summary. The member completes Part 1 of each copy before giving it to a physician.
