TRS NYC DI32: Report of Applicant's Physician

New York City teachers applying for ordinary or accident disability retirement need their physician to complete this one-page report for the TRS Medical Board. It certifies how long the applicant has been under the physician's care, their subjective and objective symptoms, diagnosis, treatment and prognosis, and states that the applicant is physically or mentally incapacitated for the performance of duty.

Who completes it

The applicant signs the authorization at the top, addressed to the physician. The applicant's physician completes the rest of the report and signs as an M.D.

Submitting and payment

How to submit it
The physician forwards the report to the TRS Medical Board; it is one of the required parts of a complete disability retirement application.

Good to know

  • The report cannot be amended once filed, so the diagnosis, treatment and prognosis should be complete before it goes to TRS.
  • A complete application also needs the member's application form, Applicant's Personal Report of Disability, DI47 health information release and up to 200 pages of medical records documenting the disability.
  • Incomplete applications are archived after three months, and the member must file a new complete application to reactivate the claim.
  • Once the file is complete, TRS schedules an interview and examination by its Medical Board, which decides eligibility. The current DI32 revision is 6/16.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can the TRS NYC Report of Applicant's Physician be changed after it is filed?

No. TRS's application instructions state that the Report of Applicant's Physician may not be amended after it is filed, even though other application details can be changed until shortly before the effective retirement date.

Where is the DI32 form found?

TRS includes DI32 inside each disability retirement application document, such as the Tier IV Ordinary Disability Retirement Application (OD4) and the accident disability applications. The member gives it to the physician, who completes it and forwards it to the TRS Medical Board.

Who decides a TRS NYC disability application?

The TRS Medical Board. Once all components are received, TRS schedules an interview and examination by the Medical Board's doctors, who determine eligibility. For Tier IV ordinary disability, the member also needs at least 10 years of total service credit.
