NYCERS Form 606: Physician's Report of Disability

New York City employees applying for disability retirement through NYCERS need their treating physician to complete this two-page report for the NYCERS Medical Board. The physician certifies the diagnosis and its duration, the accident if one caused it, objective and subjective evidence, treatment and result, and when, if ever, the member may return to the full duties of their title.

Who completes it

The member's treating physician (MD, DO, DC or similar) completes pages 1 and 2 and signs. The applicant signs the authorization for release of information at the bottom of page 2.

Submitting and payment

How to submit it
The report is returned to NYCERS with the member's disability retirement application, which members can file online through MyNYCERS with uploaded supporting documents.

Good to know

  • Form 606 is required for every NYCERS disability applicant, along with Form 608 (medical information authorization), Form 609 (applicant questionnaire) and Form 615 (release of information).
  • Objective evidence means photocopies of X-rays, EKGs, lab reports, consultant and hospital reports and pertinent physical findings; subjective evidence covers symptoms and complaints.
  • Disabling conditions not listed on the member's application are not considered, so the diagnosis on Form 606 should match the conditions claimed.
  • The NYCERS Medical Board of three physicians reviews the evidence and interviews the member. The current form is revision R06/08/11.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
