Georgia SendSS Online Notifiable Disease Reporting

SendSS, the State Electronic Notifiable Disease Surveillance System, is Georgia DPH's online alternative to the paper Form 3095. Providers required to report under Georgia Code section 31-12-2 can enter notifiable disease cases directly once they have an account. It also hosts Georgia's electronic versions of the CDC adult and pediatric HIV case report forms, called eACRF and ePCRF.

Who completes it

A health care provider or a designated reporter at the practice or facility completes the online report after registering for a SendSS account. HIV reporters need HIV user access.

Submitting and payment

How to submit it
Register or log in to SendSS from DPH's disease reporting page and enter the case online. Conditions requiring immediate reporting go by phone or fax to the District Health Office, or by phone to DPH's statewide line.
Who pays
Georgia DPH lists no fee or payment for disease reports.

Good to know

  • SendSS is DPH's electronic route for conditions reportable within 7 days; the same reports can go on Form 3095 by mail or fax to the District Health Office.
  • For HIV, Georgia's eACRF and ePCRF in SendSS are electronic versions of the CDC adult and pediatric HIV case report forms, and DPH still posts the paper forms.
  • Tuberculosis is reported through SendSS or to the district health office; Georgia does not post a separate TB case report form.
  • DPH requires reporting of both laboratory-confirmed and clinically diagnosed cases within the specified time interval.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How do I get a SendSS account in Georgia?

Use the register or log-in link for SendSS on DPH's disease reporting page. Providers who will submit HIV case reports request HIV user access, which opens the electronic adult and pediatric HIV case report forms. Until an account is active, use Form 3095 or contact the District Health Office.

Can HIV cases be reported online in Georgia?

Yes. Georgia's HIV case reporting materials offer SendSS eACRF and ePCRF as the state's electronic versions of the CDC Adult and Pediatric HIV Confidential Case Report Forms. Paper copies of the CDC forms remain available, and DPH lists a separate contact for problems reporting HIV/AIDS.
