Georgia Form 3095: Notifiable Disease/Condition Report

Form 3095 is Georgia's paper case report for notifiable diseases and conditions, required of physicians, laboratories and other health care providers under Georgia Code section 31-12-2. A clinician uses it for conditions reportable within 7 days, as an alternative to entering the case online in SendSS. It goes to the District Health Office and includes a viral hepatitis section for hepatitis A, B and C markers.

Who completes it

The diagnosing provider, or a reporter at the institution, completes the whole form and names the physician in the reporter information block. Boxes marked Local Use Only and State Use Only are left for public health staff.

Submitting and payment

How to submit it
Mail the form in an envelope marked CONFIDENTIAL to the District Health Office or fax it to the district's reporting contacts, or enter the case in SendSS instead. Immediately reportable conditions are phoned or faxed to the district reporting contacts, or phoned to DPH's statewide line.
Who pays
Georgia DPH lists no fee or payment for disease reports.

Good to know

  • Georgia requires reporting of both laboratory-confirmed and clinically diagnosed cases within the specified time interval.
  • Hepatitis results go in the viral hepatitis box: anti-HAV, HBsAg, anti-HBc and IgM anti-HBc, anti-HCV with signal-to-cutoff ratio, HCV RNA, and ALT and AST.
  • Yes/no questions flag public health risks: pregnancy, nursing home residence, daycare attendance or work, food handling, health care work, detention, outbreak link and travel in the last 4 weeks.
  • HIV/AIDS follows DPH's separate HIV reporting instructions, and cancer and benign brain and CNS tumors have their own reporting process.

The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.
