Georgia APS Online Report (Adult Protective Services)

Georgia's Division of Aging Services runs a web intake form for reporting suspected abuse, neglect or exploitation of an elder or an adult with a disability to Adult Protective Services (APS). Physicians and nurses are among Georgia's mandated reporters. The form is available 24/7, but a web report is not an accepted report for investigation until APS follows up with the reporter.

Who completes it

The reporter, mandated or not, completes the whole online intake form and should expect a callback from APS.

Submitting and payment

How to submit it
File the web report at any time; if APS has not called back within 48 hours, phone APS during business hours to follow up. Anyone in immediate danger needs 911, since APS is not a first responder.
Who pays
The Division of Aging Services lists no fee or payment for reporting.

Good to know

  • Completing the web report or leaving a voicemail does not make it an accepted report for investigation, and phone messages are returned the next business day.
  • Residents of personal care homes, community living arrangements, intermediate care facilities or skilled nursing homes are reported to DCH Healthcare Facility Regulation, not APS.
  • Georgia lists physicians, nurses, dentists, pharmacists and social workers among the professionals who must report suspected abuse of elders or adults with disabilities.
  • APS phone intake runs Monday to Friday, 8 a.m. to 7 p.m. Eastern Time; the web form is the option outside those hours.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is a Georgia APS web report enough to start an investigation?

Not on its own. The Division of Aging Services states that completing a web report or leaving a voicemail does not constitute an accepted report for investigation. If you do not get a callback within 48 hours, call APS during regular business hours to follow up.

Where do I report abuse of a nursing home resident in Georgia?

Reports about residents of community living arrangements, personal care homes, intermediate care facilities or skilled nursing homes go to the Department of Community Health, Healthcare Facility Regulation, rather than to Adult Protective Services. Call 911 first if the resident is in immediate danger.
