Tennessee PH-1600 Reportable Disease Form

Tennessee requires all health care providers and others who know of or suspect a reportable disease or event to report it to the Department of Health under Rule 1200-14-01-.02, in the time and manner set by the Commissioner's annual list. The PH-1600 is the paper route for that report, and it covers STD treatment and latent TB results too. Laboratories may also use it to report results manually.

Who completes it

The reporting provider completes the whole form: provider, patient demographics, clinical details, lab results and STD treatment fields. The patient does not complete any part.

Submitting and payment

How to submit it
Mail or fax it to the local health department, or fax it to the TDH state office; pediatric positive TST results go to the TN TB Elimination Program. Alternatives are electronic case reporting from the EHR or online entry in NBS, TDH's reportable disease system, after requesting an account through a user survey.

Good to know

  • Do not use the PH-1600 for birth defects, drug overdoses, blood lead levels, neonatal abstinence syndrome or NHSN healthcare-associated infections; those have their own reporting routes.
  • Reporting duties apply to all providers located in Tennessee and to providers elsewhere whose patients live in Tennessee.
  • The Commissioner posts the updated reportable list by November 15 each year and it takes effect January 1; the current form is REV. 12/2025 for 2026.
  • A Spanish version, Formulario de Informe PH-1600, is posted on the same TDH page, though it is a 2024 edition.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Can I report Tennessee diseases online instead of using the PH-1600?

Yes. TDH accepts reports entered online in NBS, its reportable disease system; providers request an NBS account by completing a user survey. Practices can also send reports automatically through electronic case reporting (eCR) after registering with TDH. The faxed or mailed PH-1600 remains an option.

How do I report a blood lead level in Tennessee?

Not on the PH-1600. Blood lead levels are reported to the Childhood Lead Poisoning Prevention Program by fax, through its online lead input form, or by following the program's written lead reporting instructions.

Do out-of-state providers have to report to Tennessee?

Yes. TDH says health care reporting requirements apply to all providers located in Tennessee and to providers whose patients reside in Tennessee. A clinic across the state line treating a Tennessee resident for a reportable condition reports to TDH using the PH-1600, NBS or eCR.
