Tennessee Blood Lead Level Online Report (LeadIN)

Tennessee providers who test children's blood lead, including point-of-care tests run in the office, report results to the Department of Health's Childhood Lead Poisoning Prevention Program. LeadIN is the program's online input form: no account is needed, and submitted results are reviewed and added to the LeadTRK surveillance system within two working days. Lead results must not be reported on the PH-1600.

Who completes it

The provider entering the results completes the provider block and one row per patient: demographics, address, county, sample type, collection date, funding source, device or lab and result. Laboratories report through electronic laboratory reporting instead.

Submitting and payment

How to submit it
Enter results online in LeadIN; required fields are marked in red, and several patients can be added in one submission. TDH also accepts blood lead levels by fax or by following the program's lead reporting instructions.

Good to know

  • Mark whether each sample was capillary or venous, and whether the test ran on a LeadCare II point-of-care analyzer or another device or lab.
  • The funding source field records private insurance, public coverage including Medicaid, patient pay, other or unknown for each test.
  • Providers entering results through LeadIN do not register for LeadTRK; access to LeadTRK itself is limited to staff of the Tennessee Childhood Lead Poisoning Prevention Program.
  • Blood lead levels are excluded from the PH-1600 reportable disease form, so they always go to the lead program by LeadIN, fax or its written instructions.
