Tennessee Adult Protective Services Online Report

Tennessee Adult Protective Services investigates abuse, neglect, self-neglect and financial exploitation of adults who cannot protect themselves because of a physical or mental limitation. Clinicians who see these signs can report through the One DHS Customer Portal without logging in. APS staff investigate, assess whether protective services are needed, and provide services to reduce the risk to the adult.

Who completes it

Any person, including physicians, nurses and other health care providers, completes the whole online report. Reporters can give contact details or remain anonymous.

Submitting and payment

How to submit it
Report online through the One DHS Customer Portal, with no login required, or call the APS Hotline, which answers 24 hours a day, 7 days a week. Emergencies go to 911.

Good to know

  • Each report gets a reference number that the reporter can use to return to the portal and track the report.
  • APS investigates in any living arrangement, including nursing homes, group homes, private homes and the homes of relatives or friends.
  • Residents of state-operated institutions for people with mental illness or developmental disabilities are outside APS investigation, so those concerns follow a different route.
  • APS respects the adult's right to self-determination and intervenes when that capacity is impaired and the risk of irreparable harm or death is high.

Frequently asked questions

Can I report to Tennessee APS anonymously?

Yes. TDHS says reporters may share their contact information or remain anonymous, and confidentiality remains an APS priority. Either way, each report receives a reference number that can be used to return to the portal and track it.

What does Tennessee APS count as abuse or neglect?

TDHS defines abuse as inflicting physical or mental harm or placing someone in fear of harm, including emotional, sexual and financial abuse. Neglect is a caregiver not meeting a dependent adult's basic needs; self-neglect is an adult unable to care for themselves; financial exploitation is a caregiver misusing funds meant for the adult.
