Illinois DCFS Online Child Abuse and Neglect Report

Illinois DCFS accepts non-emergency reports of suspected child abuse or neglect through an online form as well as its Child Abuse and Neglect Hotline. Health care workers are mandated reporters under the Abused and Neglected Child Reporting Act (325 ILCS 5). A clinician can use the online form when the child is not in immediate danger and none of the hotline-only situations applies.

Who completes it

The reporter, whether a mandated professional or a member of the public, completes the whole online form.

Submitting and payment

How to submit it
Submit the report on the DCFS online reporting site. For an emergency or any excluded situation, call 911 first or the DCFS Child Abuse and Neglect Hotline.
Who pays
DCFS lists no fee or payment for making a report.

Good to know

  • The online form cannot be used if the child needs immediate medical attention, is afraid to go home, is in police or physician protective custody, or has died.
  • It also cannot be used when a child is being sexually abused by an adult who will see them in the next 24 hours; call the hotline instead.
  • DCFS advises giving specific dates, times, locations and descriptions, focusing on observed facts and physical or emotional indicators rather than assumptions.
  • The Mandated Reporter Manual (CFS 1050-21) is posted in English and Spanish, and DCFS offers redesigned online mandated reporter training.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

When can't I use the Illinois online child abuse report?

DCFS lists five situations that require a call instead: the child needs immediate medical attention, is being sexually abused by an adult who will see them in the next 24 hours, is afraid to go home, is held in protective custody by a police officer or physician, or a child has died. Call 911 first if the child is in immediate danger.

Am I protected if an Illinois report turns out to be unfounded?

DCFS states that as long as a report was made in good faith, the law provides immunity from liability, even if the report is unfounded. Knowingly transmitting a false report is different: it is disorderly conduct under the Criminal Code of 2012 and a Class 4 felony.
