Illinois IDSS Online Reportable Disease Reporting

The Illinois Disease Surveillance System (IDSS) is IDPH's secure web application for reporting suspected or confirmed reportable infectious diseases. Illinois mandated reporters, including physicians, nurses, dentists and hospitals, must report these cases electronically under the Control of Communicable Diseases Code. A report entered in IDSS is routed immediately to the appropriate local health department for follow-up.

Who completes it

The health care provider, or a designated reporter with an IDPH Web Portal account, completes the whole online report with demographic, medical and exposure information.

Submitting and payment

How to submit it
Register for an account through the IDPH Web Portal, then enter cases in IDSS. Reporters without IDSS access report by mail, phone or fax to the local health department, and a short list of diseases must be phoned in immediately.
Who pays
IDPH lists no fee or payment for disease reports.

Good to know

  • Phone the local health department immediately for anthrax, foodborne botulism, diphtheria, variant influenza A, plague, polio, SARS, smallpox, suspected bioterrorism, or any unusual case or cluster.
  • Brucellosis, Q fever and tularemia also need an immediate call when bioterrorism is suspected.
  • If the local health department can't be reached, IDPH takes the call during business hours; after hours, the Illinois Emergency Management Agency duty officer contacts IDPH.
  • The Control of Communicable Diseases Code sets which diseases are reportable, the timeframes and the methods. Some IDPH pages still use the name of the earlier system, I-NEDSS.

Frequently asked questions

How do I get access to IDSS in Illinois?

IDPH says to register by going to the IDPH Web Portal and choosing Register for a Portal Account. Until access is in place, report cases by mail, telephone or fax to your local health department, or to IDPH's Division of Infectious Diseases if no local health department is available.

Does entering a case in IDSS replace a phone call?

Not for the most urgent conditions. Diseases such as anthrax, plague, smallpox and diphtheria, unusual clusters and suspected bioterrorism must be reported immediately by telephone to the local health department. Other reportable diseases follow the timeframes in the Control of Communicable Diseases Code.
