Illinois Report of Blood Lead Test Result (IDPH)

IDPH's Illinois Lead Program uses this one-page form to receive blood lead test results. A clinician meets it when the practice or facility performs the blood lead test itself, for example with point-of-care testing. It records the patient, the test date, whether the sample was venous or capillary, the result in mcg/dL, and the provider and testing facility.

Who completes it

The health care provider or testing facility that performed the test completes the whole form, including the provider's credentials and the lab ID, and prints the name of the person completing it.

Submitting and payment

How to submit it
Return the form to the Illinois Lead Program by secured email, fax or mail. Results of 35.1 µg/dL or higher are due within 24 hours, 3.5 to 35 µg/dL within 48 hours, and 0 to 3.4 µg/dL within 30 days.
Who pays
IDPH lists no fee or payment for reporting blood lead results.

Good to know

  • Every result is reportable, including results below 3.5 µg/dL, which have a 30-day deadline.
  • Pregnant patients are reported on IDPH's separate prenatal blood lead reporting form, not this one.
  • At least one race option must be selected, and the form asks for a Medicaid number if the patient has one.
  • For children, give the parent or guardian's name and phone number along with the patient's address and county.
