Good to know

  • Syphilis fields cover treponemal tests (FTA-ABS, TPPA) and non-treponemal RPR or VDRL titers, with treatment checkboxes and dates.
  • Contextual factors ask the reason for testing, sex of partners, injection drug use, date of last HIV test and whether the patient is on HIV PrEP.
  • Record whether medication was prescribed to the patient for their partners, which is how the form captures expedited partner therapy.
  • HIV is reported on the separate Adult or Pediatric HIV/AIDS case report forms, which are also faxed to the Bureau of Infectious Disease Control.
