New Hampshire Adult HIV/AIDS Case Report Form

New Hampshire providers report HIV infection or AIDS in patients older than 13 on this two-page case report form (ACRF_03_2022), as RSA 141-C and He-P 300 require. The form gathers diagnostic status, demographics, risk history, laboratory data, AIDS indicator diseases, treatment and referral details, and pregnancy information for the state HIV/AIDS Surveillance Program.

Who completes it

The current provider, or the person completing it for the facility, fills in the form except Section I, which is for HIV/AIDS Surveillance Program use only. Patient identifier information in Section II is not transmitted to CDC.

Submitting and payment

How to submit it
Fax the completed form to the Bureau of Infectious Disease Control, which takes HIV/AIDS reports on its fax line, or mail it to BIDC in Concord. Questions about HIV reporting go to the bureau by phone.
Who pays
DHHS lists no fee or billing code for the report.

Good to know

  • The demographic and patient history sections must be completed in full, including risk history before the first positive HIV test or AIDS diagnosis.
  • Section X lists AIDS indicator diseases, from candidiasis and Kaposi's sarcoma to Pneumocystis pneumonia and wasting syndrome, with diagnosis date and presumptive or definitive status.
  • The women-only section asks about OB/GYN referral, current pregnancy and due date, and the most recent live birth with hospital and child's name.
  • Patients 13 and younger are reported on the separate New Hampshire Pediatric HIV/AIDS case report form.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Vero is not affiliated with, or endorsed by, the agencies and insurers listed here. Read our editorial policy.

Frequently asked questions

Who notifies partners in New Hampshire HIV cases?

The form asks whether the patient has been informed of the infection and who will notify partners about their HIV exposure and counsel them: the health department, the medical provider, the patient, or unknown. It also records how the patient's medical treatment is primarily reimbursed, such as Medicaid/Medicare or private insurance.

Can HIV reports be faxed in New Hampshire?

Yes. The DHHS reporting page tells providers to fax reports, including HIV/AIDS, to the Bureau of Infectious Disease Control, and this form prints the same fax line. The older 2017 General Disease Reporting Form still says not to fax HIV reports, but HIV has its own form.

What facility details does the form ask for?

The facility of diagnosis, with physician, address and facility type (private physician, emergency room, counseling and testing site, hospital inpatient or outpatient, OB/GYN, health department, STD clinic, corrections or other), plus the current provider's name, facility, phone and medical record number.
