New Hampshire APS Report (NH EASY Online)

New Hampshire's Adult Protection Law (RSA 161-F:42-57) requires any person with reason to believe a vulnerable adult has been abused, neglected, exploited or is self-neglecting to report immediately to the Bureau of Adult and Aging Care Services (BAAS). The NH EASY portal lets clinicians file that report online for adults 18 and older, as an alternative to calling, emailing or faxing BAAS.

Who completes it

The reporter completes the online report in one sitting and attests that the information is truthful; reports can be anonymous on request. The adult and family complete no part.

Submitting and payment

How to submit it
File through NH EASY, or reach BAAS Central Intake by phone (weekdays 8:00 a.m. to 4:30 p.m.), email or fax. Nursing and assisted living facility residents are reported to the Long-Term Care Ombudsman, and after hours callers use 911 or local police.
Who pays
BAAS lists no fee for reporting.

Good to know

  • Proof is not required to report; BAAS reviews every report and may still assist the adult even when no abuse or neglect is found.
  • The online report must be finished in one sitting or you start over, so gather details on the adult, the person responsible, witnesses, guardians and injuries first.
  • BAAS may have to release a reporter's name if a court orders it or law enforcement requests it, though it usually keeps reporter information confidential.
  • Licensed facilities submit reports with the BAAS Facility Initial Report Transmittal Form, and abuse by paid in-home caregivers or in hospitals also goes to APS Central Intake.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is it illegal to file a false APS report in New Hampshire?

NH EASY requires the reporter to attest that the information is truthful and accurate to the best of their knowledge, and warns that it may be illegal to file a false or malicious report with Adult Protective Services. BAAS still encourages reports when you are unsure whether a situation is reportable.

What if a vulnerable adult is in immediate danger?

Call 911. NH EASY also points to 988 for a mental health crisis and to the local police department for welfare checks. BAAS phone intake runs weekdays 8:00 a.m. to 4:30 p.m., and DHHS directs callers to 911 or local police after hours, on weekends and on holidays.
