Michigan MDSS Communicable Disease Case Report

Michigan has no general paper case report form; physicians report communicable diseases through the Michigan Disease Surveillance System (MDSS), a web-based system that MDHHS says reporting can, and whenever possible should, go through. Most conditions on the 2026 reportable list must be reported to MDSS or the local health department within 24 hours of clinical or laboratory identification.

Who completes it

Physicians and laboratory directors, or their designees, must report; dentists, nurses, pharmacists, physician assistants, infection preventionists and other health professionals are also authorized to report. Office staff enter cases once the practice is enrolled.

Submitting and payment

How to submit it
Enroll through your local health department's communicable disease program or michigan.gov/mdss, then enter cases in MDSS; otherwise report to the local health department. Certain conditions are accepted only by ELR, eCR or CSV upload.
Who pays
MDHHS lists no fee or billing code; reporting is a duty under Michigan's communicable disease rules (R. 325.172 and R. 325.173).

Good to know

  • A suspected condition is reported as suspect, then reported again as confirmed to the local health department once it is confirmed.
  • Report even if you believe the report duplicates another; MDSS de-duplicates, and MDHHS says over-reporting is preferable to under-reporting.
  • A physician's report must include the patient's name, address, phone, birth date and age, sex, race and ethnicity, condition, estimated onset date, reporter identity and pertinent lab results.
  • HIV is handled separately: labs report results electronically, and providers also fax MDHHS Form 1355 to the Bureau of HIV and STI Programs.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

How do I get an MDSS account?

Contact your local health department's communicable disease program, or go to michigan.gov/mdss, to enroll. MDHHS also expects health facility infection control committees to set policies so that both the physicians treating patients at the facility and its laboratories report appropriately.

What is the reporting deadline in Michigan?

Within 24 hours for the conditions on the reportable disease list when the agent is identified by clinical or laboratory diagnosis, with footnoted exceptions. Report suspected or confirmed diseases even if you think another provider or the lab already reported them.

What goes in a Michigan outbreak report?

Unusual occurrences, outbreaks or epidemics must also be reported. To the extent the information is available, include the nature of the suspected condition, the approximate number of cases, approximate onset dates, and the location, such as a nursing home, daycare or restaurant.
