Michigan DCH-1355 Adult HIV Case Report Form

Michigan medical providers who diagnose or care for HIV in a patient older than 13 complete the DCH-1355 (MDHHS Form 1355) in addition to the lab's electronic reporting. The Bureau of HIV and STI Programs' surveillance unit uses it to record diagnosis, risk history, lab data, Stage 3 (AIDS) illnesses, pregnancy and referrals. The current Word template is revised 3-26.

Who completes it

The medical provider or facility staff completes Sections 2 to 12; Section 1 is for surveillance use only. The patient fills in nothing, though Section 12's testing and treatment history can come from a patient interview.

Submitting and payment

How to submit it
Fax the completed form, with copies of lab results if you choose, to the MDHHS Bureau of HIV & STI Programs Surveillance Unit at the fax number printed on the form.
Who pays
MDHHS lists no fee or billing code; HIV case reporting is required under Michigan rule R. 325.173 and MCL 333.5114.

Good to know

  • Section 8 notes that at least two antibody tests must be shown for an HIV diagnosis, and lists 4th and 5th generation screens, confirmatory tests, NAATs and rapid home self-tests.
  • Section 10 states that Michigan law requires providers to notify known partners or request help from the local health department, and asks who will notify them.
  • Section 11 covers current pregnancy, OB referral, due date and any recent live delivery with the child's name and delivery hospital.
  • Section 4 must be completed in full: case status (HIV or Stage 3), suspected acute infection, sex, gender identity, sexual orientation, birth country and vital status.

How this list is checked

A form is listed only after its name and number were confirmed on the issuing agency’s or insurer’s own website, or in the form itself. Each entry links to that source. The list was last checked in October 2026. Forms and their requirements change without notice, so check the official page for the current version before you complete one.

Frequently asked questions

Is the DCH-1355 needed if the lab already reported the HIV result?

Yes. Michigan's disease reporting guide says HIV lab results are reported electronically or by arrangement with MDHHS, and that the medical provider should also complete the case report form, MDHHS Form 1355, and fax it to the Bureau of HIV & STI Programs Surveillance Unit.

What if HIV lab tests are not documented?

Section 8 asks whether the HIV diagnosis has been confirmed by a clinical care provider when lab tests were not documented, and the date of that documentation. It also asks for the viral load, the CD4 count closest to the current diagnostic status and any genotype.
