Kentucky EPID 200 Reportable Disease Form

EPID 200 is the Kentucky Department for Public Health's general case report for diseases and conditions reportable under 902 KAR 2:020, and it doubles as the confidential STD case report. A clinician meets it after suspecting or diagnosing a reportable condition. It goes to the local health department serving the patient's county of residence, or to the state STD program for STD-only reports.

Who completes it

The person or agency reporting the case completes the whole form and names the attending physician. The STD section is filled in only for sexually transmitted diseases.

Submitting and payment

How to submit it
Fax or mail EPID 200 to the local health department for the patient's county, or submit electronically; conditions first reported by phone need an electronic or fax submission within one business day. STD-only reports can go directly to the state STD Prevention and Control Program.
Who pays
The Kentucky Department for Public Health lists no fee or payment for disease reports.

  • For syphilis, record the stage and any previous treatment; for gonorrhea and chlamydia, mark the infection site and any antibiotic resistance.
  • Hepatitis in pregnant women and young children uses EPID 394, and perinatal hepatitis B prevention uses EPID 399.
